It is important you are aware of your insurance benefits. Prior to making an appointment with Psychiatry Services, we encourage you to confirm your benefits at CSU Health Network. To confirm your coverage, please call the member services number listed on the back of your insurance card. In addition, please make sure you are uploading your insurance information to the CSU Student Health Portal.

Medical Services Appointments

• Walk-in appointments:
  • For conditions that demand immediate attention, such as accident, injury, or the sudden onset of severe illness
    • Monday – 8:00 am – 4:00 pm
    • Tuesday – 9:00 am – 4:00 pm
    • Wednesday – 8:00 am – 4:00 pm
    • Thursday – 8:00 am – 4:00 pm
    • Friday – 8:00 – 4:00 pm

Physical Therapy Services Appointments

• Appointments can be scheduled through your primary care provider, or by calling the Physical Therapy Department directly at (970) 491-1735
• Follow-up appointments are scheduled on the recommendations of physical therapist.

Dental Services Appointments

• Open 8 a.m. – 5 p.m., Mondays and Wednesdays, 9 a.m. – 5 p.m. on Tuesdays.
• Call (970) 491-7121 during operating hours to schedule a dental services appointment.

Optometry Services Appointments

• Open 8 a.m. – 5 p.m., Monday, Wednesday, Thursday and Friday and 9 a.m – 5 p.m. on Tuesdays.  Call (970) 491-1738 during operating hours to schedule an optometry appointment.
• If you need a contact lens refill, call (970) 491-1738 and we will coordinate that order and delivery with you over the phone.

Tobacco Cessation Appointments

• Call (970) 491-7121 during operating hours to schedule a tobacco cessation appointment.
