Thank you for scheduling a medical appointment at CSU Health Network. Please read the following information about arrival time, location, and pre-appointment tasks.
Arrival time
- Be sure to arrive for your appointment at your designated arrival time. Your arrival time was shared with you when your appointment was scheduled and is also included in the appointment reminder email or text message you received. If you are unclear about your arrival time, please call 970-491-7121.
- Late arrivals: If you arrive more than 15 minutes after to your designated arrival time, you will be asked to reschedule your appointment.
- If you need to reschedule or cancel your appointment, please call 970-491-7121.
- If you do not come to your appointment and do not cancel ahead of time, you will be charged a no-show fee.
Location
- Your appointment will take place at the CSU Health and Medical Center, located at 151 W. Lake St (northwest corner of College Avenue and Prospect Road).
- Upon arrival, go to the 2nd floor and check in at the CSU Health Network Medical Services front desk.
- Visit the Location, Hours, and Parking webpage to view a map and get more information about parking and alternative transportation.
Pre-appointment tasks
Please complete the following tasks in the Student Health Portal before arriving for your medical appointment.
- Upload a copy of your health insurance card
- Note: If you are enrolled in the CSU Student Health Insurance Plan (SHIP), you do not need to upload a copy of your health insurance card.
- Using your smart phone or other camera device, take a digital picture of your health insurance card.
- Login to the Student Health Portal using your CSU NetID (formally eName) and password.
- Select “Document Upload” in the main menu.
- In the “Document Type” drop-down menu, select “Insurance Card.”
- Select “Browse” to find the picture of your health insurance card. Select “Save” to upload it to your electronic health record.
- Complete online forms
- In the Student Health Portal, select “My Forms” in the main menu.
- Select any online forms under the “Form Name” column and complete them prior to arriving for your appointment.
