This form is only for students who meet ONE of the following criteria:

1. Domestic graduate students enrolled in continuous registration, or 1 resident instruction credit hour or more.
2. Online students (GR or UG) enrolled in 6 credit hours or more.
3. Domestic undergraduate students enrolled in a combination of six or more resident instruction and online credits
