Immunization Forms
The Following Forms can be filled out online portal.hartshorn.colostate.edu or printed out and submitted to the above address.
If a student is under the age of 18 and requests an immunization, the following form is required. This is must be printed out and faxed to (970) 491-0268.
- Authorization for Treatment of a Minor (pdf) - Required if the student is under 18 years or age.
To Submit your records to immunizations:
- Certificate of immunization for College Students (pdf) can be filled out by a health or school official. You can also submit your records, signed by a health or school authority. These can be faxed to Immunizations at (970) 491-0268 or they can be mailed in to:
